Gary L. Moffitt, M.D.
Occupational Medicine Dr. Moffitt holds board certifications in Occupational Medicine and Family Practice. He is also a Certified Medical Review Officer, Certified Independent Medical Examiner, and is an Aviation Medical Examiner. Dr. Moffitt graduated from the University of Arkansas for Medical Sciences at Little Rock in 1980 with a Doctor of Medicine Degree. His Family Practice Residency was conducted at the Area Health Education Center in Fayetteville, AR. His Occupational Medicine Training Program was by the Medical College of Wisconsin. Dr. Moffitt received a Bachelor of Science Degree in Bacteriology from the University of Arkansas in Fayetteville, AR. His interests are toxicological issues, toxicological exposures, and corporate wellness.
Konstantin Berestnev, M.D., MPH
Occupational Medicine Dr. Berestnev holds board certifications in Occupational Medicine and Family Practice. He is also a Certified Medical Review Officer and a Certified Independent Medical Examiner. He graduated from the Volgograd Medical Academy in Volgograd, Russia in 1996. Dr. Berestnev completed his family practice residency at the Area Health Education Center in Fort Smith, AR. He has completed a Master of Public Health degree from the Medical College of Wisconsin in 2005. Additionally, in 2007 he completed a residency in Occupational Medicine from the University of Pennsylvania Medical Center in Philadelphia, Pennsylvania. Dr. Berestnev's interests are toxicology, screening programs, commercial driver evaluations, and industrial hygiene.

Jonathan W. Lee, P.T.,C.E.A.S. Physical Therapy Mr. Lee has practiced physical therapy in northwest Arkansas since he received his degree in physical therapy from the University of Central Arkansas in 1996. His experience in physical therapy includes orthopedic/ sports medicine, outpatient rehabilitation, and post-surgical rehabilitation. He received his certification as an Ergonomic Assessment Specialist (C.E.A.S.) through the Back School of Atlanta in 2003 and is also certified to perform functional capacity evaluations (FCEs) through Roy Matheson and Associates. Mr. Lee affiliates with Northwest Arkansas Community College and the University of Central Arkansas as a clinical affiliation site for physical therapy assistant and physical therapy students in addition to providing similar services for Master’s level athletic training students from the University of Arkansas.
